Program description
The programme is implemented under the PRFU Multi-Donor Programme, funded by the governments of Canada, Estonia, Finland, Norway, Sweden, Switzerland and the United Kingdom. It supports the Government of Ukraine, local communities and civil society in the context of Russia’s aggression.
The programme focuses on social protection and on strengthening communities’ capacity to provide accessible and well-coordinated social services.
The expected results include:
- strengthening the capacity and effectiveness of authorities and partners to respond to people’s needs;
- supporting the capacity of the Ukrainian authorities to restore and improve priority service delivery in communities;
- strengthening horizontal and vertical social cohesion;
- collecting and sharing data and experience to better reflect local needs in policy and programme planning.
Activity Objective
Under the PRFU programme, one grant is envisaged for the development, validation and pilot implementation of an Individual Care Planning (ICP) tool for social service provision.
The activity is based on existing needs-assessment and individual social service planning processes and aims to support:
- the transition from needs assessment to individual social service planning;
- identification of a person’s life situation, goals and needs;
- development of an individual package of support measures;
- monitoring progress and reviewing plans based on the results.
The grant project will also include practical testing of the methodology and tools, training of managers and social workers, piloting in territorial communities, and preparation of recommendations for further scale-up.

Eligibility
Eligibility Criteria and Participation Requirements
Organisations submitting project proposals must meet the established requirements. In particular, they must:
- have the status of an officially registered and recognised civil society organisation or NGO under Ukrainian law;
- have non-profit status;
- be prepared to undergo the PRFU due diligence procedure and complete the self-assessment form. Applicants are screened through the Visual Compliance system;
- have qualified personnel with experience in project management and financial reporting;
- demonstrate an effective governance structure and management systems, including financial, administrative and technical policies and procedures;
- demonstrate readiness to comply with health, safeguarding and human rights policies;
- ensure appropriate duty of care for their personnel and engaged consultants and follow the relevant procedures;
- comply with ethical conduct requirements and the «Chemonics» Supplier Code of Conduct;
- submit only one application under this CFP. If two or more applications are submitted, PRFU reserves the right to consider only the first application.
Organisations Not Eligible to Apply
The following organisations are not eligible to submit applications:
- organisations registered in the temporarily occupied territories of Ukraine that have not changed their registration to a territory controlled by the Government of Ukraine;
- organisations not registered under Ukrainian law, unless otherwise specifically agreed with the FCDO;
- organisations that have misused donor funds during the last five years;
- organisations listed in additional UK Government sources concerning prohibited organisations or sanctioned entities;
- organisations with an active «exclusion from participation» status in Visual Compliance;
- political parties, groups, institutions, or their subsidiaries and affiliated structures;
- religious organisations whose primary purpose is religious activity;
- organisations that support or promote anti-democratic policies or illegal activities;
- organisations registered in Russia, North Korea or Iran;
- organisations associated with any person or organisation included in the State Register of Sanctions of the National Security and Defence Council of Ukraine (NSDC).
Financing
The programme provides for 1 grant. Funding may be provided in two forms:
- a grant based on reimbursement of costs;
- a grant based on a fixed amount for eligible milestones, involving periodic financial reporting.
The expected maximum grant amount is up to UAH 15,290,000. However, the final amount will depend on the planned activities and actual project needs and may be lower than the stated range.
The grant is subject to an external audit, with the audit costs covered by PRFU. The project must therefore include the relevant audit costs in its budget.
Cost Eligibility Principles
Budgeted costs must:
- be directly connected with the implementation of the activities and necessary for their execution;
- comply with applicable Ukrainian legislation, including taxation, labour and other relevant requirements;
- be reasonable, justified and consistent with the principles of sound financial management;
- be incurred within the specified implementation period;
- be identifiable, supported by documentation and recorded in the organisation’s accounting records in accordance with applicable standards and usual accounting practices.
The purchase of goods or services provided by companies that have received funding or other assistance from governments or the European Commission, or have been suspended or excluded from receiving such assistance or funding, is not permitted.
Procurement of goods manufactured by Huawei Technology Company, ZTE Corporation, Hytera Communications Corporation, Hangzhou Hikvision Digital Technology Company, Dahua Technology Company and Hangzhou Hikvision Digital Technology Company is also prohibited.
Supported Activities
The grant programme supports projects aimed at the development, piloting and implementation of an Individual Care Planning (ICP) methodology for the provision of social services in territorial communities in Ukraine.
Key supported activities include:
- Preparatory stage — development of an analytical evidence base, mapping of social services and additional support, and analysis of existing case-management practices and mechanisms.
- Development of the ICP toolkit — development of the methodology and individual care-planning tools, performance indicators, monitoring mechanisms and informed-consent procedures.
- Training and capacity building — preparation of a training and methodological package and training for social service professionals on the practical application of the ICP methodology.
- Pilot implementation — application of the ICP tools with actual social service recipients in 12 target territorial communities.
- Validation and refinement — collection of feedback from service recipients and professionals, analysis of pilot results, refinement of the tools and preparation of materials for further use and dissemination.
The project also includes the preparation of methodological, training, analytical and practical materials, including a scaling roadmap, policy recommendations and a formal handover protocol for the ICP toolkit.
Learning during implementation is also supported through After Action Reviews and Action Learning Sessions, aimed at identifying lessons learned, sharing good practices and improving approaches.
Roadmap
The programme includes the following key stages and dates:
- 29 July 2026 — programme announcement date.
- 4 September 2026, 23:59 Kyiv time — deadline for submitting questions.
- 7 August 2026, 11:00 Kyiv time — online pre-application workshop on Microsoft Teams. The workshop will provide an opportunity to ask questions about the CFP and receive guidance on completing the required documents.
- 8 September 2026, 23:59 Kyiv time — application submission deadline.
- 9 months — expected grant duration.
How to Apply
To participate in the competition, the applicant must prepare and submit a complete project proposal in accordance with the established requirements:
- Complete the online form available via the provided link. The form contains basic information about the organisation and the project proposal.
- Prepare the project proposal in Ukrainian in Word format, using Times New Roman, font size 11, with single line spacing. The proposal must be prepared based on the templates provided in Annexes A, B, C and D.
- Submit the complete project proposal electronically to pfru-grants@chemonics.com, indicating the competition number 19-11_Individual Care Plan.
- The application must be submitted no later than 23:59 Kyiv time on 8 September 2026.
- All required documents must be submitted by the established deadline. PFRU reserves the right not to consider late applications or applications that do not meet the eligibility criteria.
Evaluation Criteria
Before evaluation, applications undergo an eligibility and compliance check, including verification of:
- submission within the established deadline;
- the applicant organisation’s official registration and recognition under Ukrainian legislation;
- compliance of the proposed activities with the defined geographical priorities;
- screening of the organisation and its personnel through the Visual Compliance and Touscontrol systems;
- inclusion of all supporting documents.
Applications meeting the eligibility criteria are submitted to the Competition Committee. Full grant applications are assessed using a 100-point scoring system:

Project methodology and design — 40 points. The assessment considers the quality and realism of the proposal, the appropriateness of the methodology, and the relevance of integrated GEDSI approaches addressing gender equality, disability inclusion and social inclusion.
Expertise and experience — 30 points. The assessment considers relevant previous work, contact details for references and CVs of the proposed personnel.
Budget and value for money — 20 points. The budget is assessed for its justification, clarity, realism and optimal use of resources.
Risk management — 10 points. The assessment considers the structure and content of the risk matrix and measures designed to minimise potential budgetary, environmental and security risks.
The individual criteria use different scoring levels, including 40/30/20/10/0, 30/20/10/0, 20/10/0 and 10/5/0 points respectively.
Applications receiving the highest scores, subject to achieving the minimum threshold of 70 points, may proceed to negotiations and an additional due diligence process.
Required Documents
To participate in the competition, the applicant must submit the complete set of documents and forms required under the call:
- Completed online application form, available via the relevant link. It contains basic information about the organisation and the project proposal.
- Grant Proposal Form (Annex A), signed by an authorised representative of the applicant. The applicant must describe the project implementation strategy, including the requirements of Section 2.3 “Detailed Description of Activities”, and demonstrate an understanding of PFRU programme needs.
- Budget (Annex B) — submitted in the prescribed format and in accordance with the relevant instructions and recommendations.
- Applicant Self-Assessment Form (Annex C). The available supporting documents should be attached, including:
- extract from the Unified State Register or re-registration documents, where applicable;
- non-profit status certificate;
- articles of association/statutes (latest version);
- organisational structure and CVs of key personnel;
- available policies;
- financial statements for the last three years, if available;
- audit report(s), if available.
- Risk Matrix (Annex D).
- Letters of recommendation from administrations — where possible, preferably provided.
The grant proposal must be submitted in Ukrainian, in Word format, using Times New Roman, font size 11, single line spacing, and the templates provided in Annexes A, B, C and D.
Updates / Announcements
The programme provides for an online workshop before the application submission.

- Purpose: participants will have an opportunity to ask questions regarding the CFP and receive guidance on completing the application form, self-assessment form, and other requirements specified in the CFP.
PFRU intends to process applications in a timely manner. Participants who successfully pass the evaluation will be notified within 20 working days that they have been recommended for a grant award.
However, a grant award recommendation does not constitute a funding decision and does not oblige Chemonics to enter into a grant agreement.
The final decision will depend on the completion of the required procedures, including:
- Due Diligence review;
- screening through the Visual Compliance system;
- approval of the budget proposal;
- where required, obtaining approval before the final funding decision.
Reporting and Compliance
The grant may be provided as a cost-reimbursement grant or a fixed-amount grant based on eligible milestones, depending on the programme conditions.
Budgeted costs must be eligible in accordance with the FCDO Eligible Cost Guidance for Commercial Contracts and the applicable FCDO Eligible Cost Guidance for Grant Agreements with mandatory reporting.
A specific financial reporting requirement also applies: the grantee receives the final payment of 5–10% of the total project budget only after submitting 100% of the required reporting, including supporting evidence for all financial transactions.
Legal Terms
PFRU grants are provided in accordance with the Standard Terms and Conditions of the UK FCDO, as well as the Chemonics Supplier Code of Conduct (FCDO Code of Conduct). These requirements are incorporated into the Applicant Self-Assessment Form and are mandatory for all funding recipients.
PFRU ensures that organisations funded by the UK FCDO comply with the requirements set out in these provisions in accordance with the terms of the grant agreements.
The UK FCDO reserves the right to refuse to award a grant to an applicant, withdraw Chemonics’ offer, and terminate, fully or partially suspend, or otherwise discontinue the grant award at any time.
Submission of an application does not oblige Chemonics to finance any activity or provide funding to any applicant.
